Job summary

Neighborhood Healthcare seeks a Physical Therapist to provide rehabilitative PT evaluations and plans for geriatric participants. You will integrate PT plans into the overall POC under provider orders, supporting home-bound and facility-based participants.

Role requires CA PT license, 1 year geriatric PT experience, and BLS/CPR-First Aid. A $5,000 signing bonus is offered, with a pay range reflecting hourly rates converted to annual compensation.

Qualifications

• Valid CA Physical Therapist license in good standing
• One year PT geriatric experience; three years preferred
• Bilingual English/Spanish preferred
• Current BLS certification required; CPR/First Aid certification required for all staff

Responsibilities

• Screen and evaluate rehab needs per APTA guidelines
• Collaborate with nursing home and assisted living staff
• Document POCs for referrals to other services (OT, ST, medical clinic)
• Provide PT treatments, including modalities and therapeutic exercise
• Instruct on DME use and maintenance; provide home exercise guidance
