Job summary

myPlace Health is seeking a Physical Therapist for our PACE program in California. You will work on a collaborative Interdisciplinary Team to create personalized care plans and deliver life-changing therapeutic care at our PACE center.

Lead exercises, gait training, and home assessments to restore mobility and independence. You will educate participants and families to sustain progress at home, under physician orders and IDT guidance.

Qualifications

• Graduation from a Physical Therapy program accredited by the American Physical Therapy Association.
• Active California licensure in good standing with the state board.
• CPR and First Aid certification within 60 days of employment.
• Ability to lift, carry, or move up to 50 pounds.
• Onsite schedule: 100% onsite, Monday through Friday.

Responsibilities

• Collaborate with the IDT to complete assessments, develop individualized care plans, and review service determinations.
• Assess mobility levels and functional abilities to create tailored therapy plans.
• Deliver therapeutic exercises, gait training, and ambulation support to help participants achieve goals.
• Develop engaging individual and group exercise programs focused on maintenance and rehabilitation.
• Provide interventions in line with physician orders and care plans, with accurate documentation.
• Identify priority areas and recommend follow-ups as needed.
• Refer for additional services or resources when appropriate.
• Educate participants and families to sustain progress at home.
• Manage therapy equipment to ensure safety and program efficiency.
• Collaborate with nursing homes, SNFs, and assisted living staff on rehab needs.
• Lead home assessments and deliver in-home treatments.
