Neighborhood Healthcare offers a Physical Therapist role in Riverside, CA, providing licensed PT services to seniors in a PACE setting. The role includes clinical evaluations, treatment plans, and integration of PT plans into the overall plan of care.

The position operates Monday–Friday, 8:00 am–4:30/5:00 pm, with a $5,000 signing bonus and a strong benefits package including partially company-paid medical, dental, and vision plans, vacation, holidays, sick time, and a 403b retirement plan.

Qualifications

• CA Physical Therapist license required and maintained
• One year geriatrics experience preferred
• Current BLS certification required upon hire and maintained

Responsabilités

• Screen and evaluate rehabilitation needs for acute, restorative, or maintenance levels
• Collaborate with nursing home staff on rehabilitation plans
• Document POC and referrals to related services
• Communicate participant needs to staff and family
• Provide physical therapy treatments and home-use instructions
• Ensure DME knowledge and loan program understanding
• Delegate plans to PT support team per patient needs
• Attend interdisciplinary team meetings and QM activities
• Maintain and document all procedures in patient charts
