Job summary

Neighborhood Healthcare is seeking a licensed Physical Therapist to provide evaluations and treatment plans for PACE participants at our Riverside County centers. The role supports acute, restorative, or maintenance therapy, integrating PT into the plan of care as directed by providers.

Schedule is Monday–Friday 8am–5pm; includes a $5,000 signing bonus. You will screen participants, collaborate with nursing and staff, document plans, deliver therapies, and educate patients and families.

Qualifications

• Valid CA Physical Therapist license in good standing.
• One year of physical therapy experience in geriatric settings; two years preferred.
• Bilingual English/Spanish preferred.
• Current Basic Life Support (BLS) certification required.
• CPR and First Aid certifications required for all PACE employees.

Responsibilities

• Screen and evaluate participants for rehabilitation needs.
• Collaborate with nursing home and staff regarding rehabilitation needs.
• Document participant POCs and referrals to other services.
• Provide physical therapy treatments including modalities and therapeutic exercise.
• Instruct participants and caregivers in DME use and home exercises.
• Perform home visits and educational sessions as needed.
• Coordinate with multi-disciplinary team to integrate services.
