Job summary

Neighborhood Healthcare seeks a licensed Physical Therapist to support the PACE program with rehabilitative evaluations and treatment plans. You will integrate PT into the plan of care and coordinate with the interdisciplinary team to optimize client health and independence.

The role follows a Monday–Friday 8am–5pm schedule in California, and includes a $5,000 signing bonus along with a generous benefits package. You will conduct home visits and contribute to care transitions as needed.

Qualifications

• CA Physical Therapist license is required.
• BLS certification required upon hire and must be current.

Responsibilities

• Screen and evaluate participants for rehabilitation needs (acute, restorative, or maintenance).
• Collaborate with nursing home and assisted living staff on rehabilitation plans.
• Document therapy plans and participate in interdisciplinary POCs and team reviews.
• Provide PT treatments, including modalities, therapeutic exercise, and mobility training.
