Job summary

Fusion Physical Therapy Partners seeks a Per Diem Physical Therapist to join Engage Senior Therapy, delivering compassionate geriatric care at assisted living facilities in Bedford, NH. You will evaluate patients, create personalized treatment plans, and support independence for seniors within a collaborative team.

Qualifications

• Graduation from an accredited physical therapy program.
• Active or pending state PT license required.
• Demonstrated passion for delivering quality, evidence-based patient care.
• Interest in working with the senior population.

Responsibilities

• Evaluate patients to assess strength, balance, mobility, and impairments.
• Develop and implement personalized treatment plans to improve mobility and independence.
• Deliver care at one or more assisted living facilities with a high level of independence.
• Educate patients and families on home exercise programs and strategies.
• Document care in EMR in compliance with facility standards.
• Engage in ongoing professional growth through educational programs and workshops.
