Job summary

Fusion Physical Therapy Partners’ Engage Senior Therapy division in Concord, NH seeks a dedicated Per Diem Physical Therapist to serve seniors in assisted living facilities. You will evaluate patients, create and adjust treatment plans, and deliver high-quality care with autonomy while supported by the Engage team.

You’ll educate patients and families on home exercise programs, document care in EMR, and participate in ongoing education to grow professionally.

Qualifications

• Graduation from an accredited physical therapy program.
• Active or pending state license to practice PT.
• Interest in working with the senior population.
• Superior customer service and interpersonal communication skills.

Responsibilities

• Evaluate patients to assess strength, balance, mobility, and functional impairments
• Develop and implement personalized treatment plans that focus on improving mobility, reducing pain, and enhancing overall function
• Deliver care at one or more assisted living locations, providing meaningful services to a diverse group of seniors
• Provide care with a high level of independence in assisted living communities, while always supported by the broader Engage team for guidance and resources
• Educate patients and families on home exercise programs and strategies for maintaining functional independence and a better quality of life
• Document patient care services in EMR
• Enhance professional growth and development through participation in educational programs, in-service meetings, and workshops
