Fusion Physical Therapy Partners (Engage Senior Therapy) seeks a Per Diem Physical Therapist to provide high-quality geriatric care at assisted living facilities in Bedford, NH. You will evaluate patients, develop individualized plans, and deliver care with a high degree of independence while collaborating with a supportive team.

The role emphasizes compassionate patient education, documentation in EMR, and ongoing growth through educational programs and CEUs.

Qualifications

• Graduation from an accredited physical therapy program.
• Active state license to practice PT (or pending).
• Interest in working with the senior population.
• Superior customer service and interpersonal communication skills.
• Strong communication and interpersonal skills to work with patients, families, and colleagues.

Responsibilities

• Evaluate patients to assess strength, balance, mobility, and impairments.
• Develop and implement personalized treatment plans to improve mobility and function.
• Deliver care at one or more assisted living locations.
• Provide care with independence, while supported by the Engage team.
• Educate patients and families on home exercise programs and independence strategies.
• Document patient care in EMR in compliance with standards.
• Support professional growth through education and workshops.
