Fusion Physical Therapy Partners in Massachusetts is seeking a compassionate Physical Therapist to join Engage Senior Therapy. You will serve a geriatric population across independent living, assisted living, dementia, and memory care settings, delivering evidence-based care and fostering meaningful patient relationships.

Responsibilities include comprehensive evaluations, personalized treatment plans, and hands-on interventions with a focus on mobility, independence, and safety.

Qualifications

• CAPTE-accredited PT program completion is required.
• Active state PT license in good standing or licensure-eligibility.
• Passion for compassionate, evidence-based geriatric patient care.

Responsibilities

• Conduct comprehensive evaluations of strength, balance, mobility, pain, and function.
• Develop and implement individualized treatment plans to improve mobility and reduce pain.
• Deliver hands-on therapies: therapeutic exercise, balance and gait training, neuromuscular re-education, manual therapy.
• Promote safety and fall prevention through assessments and education.
• Provide care across assisted living communities with strong communication to families and teams.
• Educate residents and caregivers on home exercise programs for independence.
• Maintain accurate documentation and progress using EMR per standards.
• Collaborate with therapists and support staff for coordinated care.
• Participate in ongoing professional development and mentorship.
