Qualifications

• Graduation from an accredited physical therapy program.
• Active or pending state PT license.
• Passion for geriatric patient care.
• Strong customer service and interpersonal skills.
• Ability to work independently with team support.

Responsibilities

• Evaluate patients to assess mobility and function.
• Develop and implement personalized treatment plans.
• Deliver PT care at assisted living facilities.
• Educate patients and families on home exercise programs.
• Document care in EMR according to standards.
• Support ongoing professional growth through training.
