Job summary

Engage Senior Therapy in Concord, NH is seeking a dedicated Per Diem Physical Therapist to provide on-site rehab services at assisted living facilities across our region. You will deliver compassionate, patient-centered care to seniors and support mobility and independence.

As a PT, you will evaluate patients, develop and implement treatment plans, document care in EMR, educate patients and families on home exercise programs, and collaborate with the Engage team for guidance.

Qualifications

• Graduation from an accredited physical therapy program.
• Active or pending state license to practice PT.
• Interest in working with the senior population.
• Superior customer service and interpersonal communication skills.
• Strong communication and interpersonal skills to work effectively with patients, families, and colleagues.

Responsibilities

• Evaluate patients to assess strength, balance, mobility, and functional impairments.
• Develop and implement personalized treatment plans to improve mobility and independence.
• Deliver care at one or more assisted living locations.
• Document patient care services in EMR.
• Educate patients and families on home exercise programs.
