Job summary

Engage Senior Therapy is seeking a dedicated Per Diem Physical Therapist to join our team delivering on-site therapy at assisted living facilities in Bedford, NH. You will evaluate patients, develop and implement personalized treatment plans, and provide care with independence, supported by our Engage team.

Qualifications

• Graduate from an accredited physical therapy program.
• Active or pending state PT license.
• Interest in working with the senior population.
• Superior customer service and interpersonal skills.

Responsibilities

• Evaluate patients to assess strength, balance, mobility, and impairments.
• Develop and implement personalized treatment plans to improve mobility and function.
• Deliver care at one or more assisted living locations.
• Provide care with independence while receiving team guidance when needed.
• Educate patients and families on home exercises for independence.
• Document patient care services in EMR compliant with state standards.
• Engage in educational programs and workshops for professional growth.
