Job summary

Telos Health Systems is seeking a Psychotherapist (LMHC, LCSW, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services in Skilled Nursing Facilities and Assisted Living Facilities. The role offers flexibility, clinical autonomy, and strong administrative and leadership support.

Located in Stuart and Palm Bay, FL, this part-time position allows you to contribute to high-quality patient care while maintaining a balanced schedule.

Qualifications

• Active Florida license as LMHC, LMFT, or Psychologist.
• Master's or doctoral degree in a relevant mental health field.
• Ability to work independently while collaborating within a clinical team.
• Strong communication and clinical documentation skills.
• Level II background check per AHCA requirements.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with a multidisciplinary care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff, medical providers, and other professionals.
• Adhere to federal, state, and organizational standards, policies, and regulations.
