Job summary

Telos Health Systems is seeking a Psychotherapist to provide mental health evaluations and psychotherapy in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.

The position supports a 1 day per week schedule with hybrid charting and collaboration with a multidisciplinary team to deliver high-quality geriatric care.

Qualifications

• Active Florida license as LMHC, LMFT, or Licensed Psychologist (PsyD/Ph.D.).
• Master's degree (LMHC/LMFT) or Doctoral degree (Psychologist).
• Ability to work independently while collaborating within a clinical team.
• Strong communication and clinical documentation skills.
• Experience working with geriatric populations.
• Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff, medical providers, and other healthcare professionals.
• Adhere to all federal, state, and organizational standards, policies, and regulations.
