Telos Health Systems is seeking a Psychotherapist (LPCC, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to perform mental health evaluations and provide psychotherapy in Skilled Nursing Facilities and Assisted Living Facilities. The role offers flexible scheduling, clinical autonomy, and strong administrative support.

Qualifications

• Active North Carolina license in LPCC, LMFT, or Psychologist (PsyD/Ph.D.)
• Experience with geriatric populations in SNF/AL settings
• Strong ability to work independently within a clinical team

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop and update individualized treatment plans with the care team.
• Maintain compliant clinical documentation.
• Coordinate care with facility staff and other health professionals.
• Adhere to applicable standards, policies, and regulations.
