Job summary

Telos Health Systems is seeking a Psychotherapist to provide mental health evaluations and psychotherapy services within Skilled Nursing Facilities and Assisted Living Facilities. Flexible scheduling, clinical autonomy, and strong administrative support are offered.

Qualifications

• Active North Carolina license as LPCC/LMFT or Psychologist.
• Master’s or Doctoral degree in a relevant mental health field.
• Ability to work independently while collaborating with a clinical team.
• Strong communication and clinical documentation skills.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with a multidisciplinary team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff, medical providers, and other professionals.
• Adhere to federal, state, and organizational standards and policies.
