Job summary

Telos Health Systems is seeking a Psychotherapist (LMHC, LCSW, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.

This part-time position (2 days per week, pay-per-encounter) allows independent practice within a collaborative clinical team, with extensive benefits

Qualifications

• Active Florida license (LMHC, LMFT, or Psychologist PsyD/Ph.D.)
• Master’s degree (LMHC/LMFT) or Doctoral degree (Psychologist) required
• Ability to work independently while collaborating with a clinical team
• Level II background check required per AHCA

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop and update individualized treatment plans with a multidisciplinary team.
• Maintain accurate, timely, compliant clinical documentation.
• Coordinate care with facility staff and healthcare professionals.
• Adhere to federal, state, and organizational standards and policies.
