Job summary

Telos Health Systems is seeking a Psychotherapist ( LPCC, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.

Qualifications

• Active North Carolina license required (LPCC/LMFT/Ph.D./PsyD).
• Master's or Doctoral degree in a relevant mental health field.
• Ability to work independently within a clinical team.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with a care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff and other health professionals.
• Adhere to federal, state, and organizational standards and policies.
