Telos Health Systems is seeking a Psychotherapist (LMHC, LMFT, or Licensed Psychologist - PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.

The position requires licensure in Florida and involves a hybrid schedule across Trinity, FL and St. Petersburg, FL.

Qualifications

• Active Florida license LMHC/LMFT/Psychologist
• Master’s or Doctoral degree in mental health field
• Experience in geriatric, SNF/AL settings

Responsibilities

• Conduct comprehensive mental health assessments and evaluations.
• Provide individual psychotherapy and treatment plans.
• Coordinate care with facility staff and healthcare professionals.
• Maintain compliant clinical documentation.
