Job summary

Telos Health Systems is a clinical service organization delivering mental health care to residents in Skilled Nursing and Assisted Living facilities. We seek a Psychotherapist (LCSW, LPC, LMFT, PsyD, or PhD) to perform evaluations and psychotherapy with strong administrative and leadership support.

Offering a sign-on bonus up to 10,000 dollars, the role supports flexible scheduling with clinical autonomy. Full-time W-2 employment, RVU-based compensation, and comprehensive benefits are provided.

Qualifications

• Active Pennsylvania license as an LCSW, LPC, LMFT, or Licensed Psychologist (PsyD/PhD).
• Master's degree (LCSW, LPC, LMFT) or Doctoral degree (Psychologist) in a relevant mental health field.
• Ability to work independently while collaborating effectively within a clinical team.
• Strong communication and clinical documentation skills.
• Preferred: experience with geriatric populations.
• Preferred: experience in Skilled Nursing, Assisted Living, or long-term care settings.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with the multidisciplinary care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff, medical providers, and other healthcare professionals.
• Adhere to all federal, state, and organizational standards, policies, and regulations.
