Job summary

Telos Health Systems is seeking a Psychotherapist (LCSW, LCMHC, LMFT, Psychologist - PsyD/Ph.D.) to provide mental health evaluations and psychotherapy in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.

Locations include Sparta, NC with additional days in Charlotte, Mooresville, Salisbury, Clemmons, Lexington, Wilkesboro, Yanceyville, Henderson, Gatesville, and Wilmington, NC.

Qualifications

• Master's or doctoral degree required in a mental health field.
• Active, unrestricted license in the state of practice.
• Ability to work independently while collaborating in a clinical team.
• Strong communication and clinical documentation skills.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with a multidisciplinary care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff, medical providers, and healthcare professionals.
• Adhere to all federal, state, and organizational standards, policies, and regulations.
