Job summary

Telos Health Systems is seeking a Psychotherapist (LCSW, LPCC, LMFT, or Licensed Psychologist – PsyD/Ph.D.) to deliver mental health evaluations and psychotherapy in Skilled Nursing and Assisted Living facilities across KY. Flexible scheduling, clinical autonomy, and strong administrative support are offered.

The role supports geriatric populations with a focus on collaborative care and compliance, with hybrid scheduling options and a comprehensive benefits package.

Qualifications

• Active Kentucky license in LCSW, LPCC, LMFT or Psychologist (PsyD/Ph.D.).
• Master’s degree (LCSW/LPCC/LMFT) or Doctoral degree (Psychologist).
• Ability to work independently within a clinical team.
• Strong communication and clinical documentation skills.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans with a multidisciplinary team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff, medical providers, and other professionals.
• Adhere to federal, state, and organizational standards.
