Job summary

TEEMA is hiring a Physical Therapist to assess and treat elderly patients in the Fresno area, splitting time between clinic and field.

You will develop individualized care plans, educate patients and families, and collaborate with a multidisciplinary team to promote independence and safety at home. A California license and geriatric experience are preferred. Training is available for candidates with relevant experience.

Qualifications

• Advanced degree in Physical Therapy from an accredited school of Physical Therapy.
• Current Physical Therapist license in the state of practice.
• Two (2) years of experience in a team-based hospital, nursing home, or community-based setting preferred.
• One (1) year of experience working with a frail or elderly population preferred; training may be provided on hire.

Responsibilities

• Completing full patient assessments.
• Creating individualized treatment plans.
• Providing treatment focused on chronic conditions common in geriatric populations.
