Job summary

TEEMA is seeking a Physical Therapist to plan, organize, and deliver high-quality PT services across clinics, centers, and patient homes. The role emphasizes assessment, education, and collaboration to promote independence and safety for older adults and those with complex needs.

The ideal candidate holds an advanced PT degree, current licensure, and strong skills in home safety, adaptive equipment, and interdisciplinary teamwork.

Qualifications

  • Advanced degree in Physical Therapy from an accredited institution.
  • Current, active license to practice Physical Therapy in the state of employment.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
  • Medically cleared for communicable diseases and current on all required immunizations prior to patient contact.
  • Successful completion of a comprehensive background check, including criminal history, license verification, education verification, and drug screening.

Responsibilities

  • Provide exceptional physical therapy care as an active and collaborative member of the team.
  • Conduct in-person assessments for patients at initial enrollment and as needed, based on team determinations.
  • Evaluate and recommend adaptive and durable medical equipment, specialized footwear, and prosthetics; assess home safety and mobility, recommending modifications and equipment as appropriate.
  • Design and implement physical therapy treatment programs consistent with professional standards and organizational policies.
  • Deliver therapeutic interventions aimed at achieving individualized goals; collaborate with the team on patient care planning.
  • Accurately document evaluations, assessments, treatment plans, and outcomes in a timely manner.
