TEEMA is seeking a licensed Physical Therapist to plan, implement, and coordinate high-quality therapy services for patients across clinics, centers, and home visits in Fresno, California. Qualifications

Advanced degree in Physical Therapy from an accredited institution.
Current, active license to practice Physical Therapy in the state of employment.
Valid driver's license and reliable transportation.
Successful completion of a comprehensive background check and drug screening.

Responsibilities

Provide exceptional physical therapy care as an active and collaborative member of the team.
Conduct in-person assessments at enrollment and as needed.
Evaluate adaptive equipment and assess home safety and mobility needs.
Design and implement PT treatment programs aligned with standards.
Deliver interventions to achieve individualized goals and document outcomes.
Educate patients, families, and caregivers on home programs and equipment use.
Document evaluations, plans, and outcomes in a timely manner.
Participate in daily team meetings and coordinate care planning.
