Job summary

Qualifications

• Active California Physical Therapy license or eligibility.
• Advanced degree in Physical Therapy (DPT) from an accredited program.

Responsibilities

• Assess participants at enrollment and as needed; develop and implement personalized treatment plans.
• Evaluate need for adaptive equipment and home safety modifications; recommend solutions.
• Administer therapy, document progress, adjust plans to meet goals.
• Educate participants and families on procedures, home programs, and safety techniques.
• Train staff on body mechanics, equipment usage, and safety to maximize independence.
• Collaborate with interdisciplinary team to develop and update care plans.
• Participate in team meetings and quality improvement; maintain accurate documentation.
• Provide services in clinics, homes, and community locations.
