TEEMA is seeking a licensed Physical Therapist to support elderly and disabled patients across outpatient centers in Long Beach, CA. The role blends direct clinical care with organizational leadership to improve patient outcomes while developing staff and ensuring compliance across sites.

Responsibilities include evaluating patients, creating care plans, and providing therapy in multiple centers, with opportunities to lead CQI initiatives and participate in interdisciplinary committees.

Qualifications

• Active Physical Therapist license issued by the State of California.
• Minimum of 1 year of experience with elderly or disabled populations.
• Current Basic Life Support (BLS) certification.

Responsibilities

• Provide high-quality physical therapy services to restore function and mobility.
• Develop personalized care plans to promote independence.
• Deliver therapy in-person or remote to support staffing and compliance.
• Participate in team meetings and coordinate care with interdisciplinary staff.
• Lead staff onboarding, training, and ongoing professional development.
• Contribute to data collection, compliance documentation, and CQI efforts.
• Prepare and present program planning and quality improvement initiatives.
• Collaborate with quality teams and department leads to align services.
