Job summary

TEEMA is seeking a compassionate Physical Therapist to assess, plan, and deliver high-quality therapy across clinics, homes, and contracted facilities. The role focuses on independence, safety, and optimized mobility for older adults and complex cases.

The candidate applies evidence-based rehabilitation, collaborates with an interdisciplinary team, and educates patients and families on home programs. A valid PT license and current immunizations are required.

Qualifications

• At least one year of experience with frail or elderly populations preferred.
• 1) Licensed to practice Physical Therapy in the employing state.
• Valid driver's license and reliable transportation.
• Medically cleared for communicable diseases and required immunizations.
• Comprehensive background check including license, education, and criminal history.

Responsibilities

• Provide high-quality physical therapy care as part of a collaborative team.
• 2) Conduct in-person assessments at enrollment and as needed.
• Evaluate adaptive equipment and home safety, recommending mods as appropriate.
• Design and implement treatment programs aligned with professional standards.
• Deliver interventions to achieve individualized goals with team collaboration.
• Document evaluations, plans, and outcomes accurately and timely.
• Measure treatment effectiveness and revise plans as needed.
• Educate patients, families, and caregivers on home programs and devices.
• Train staff on body mechanics, transfers, mobility, and safety.
