Job summary

TEEMA is seeking a licensed Physical Therapist to support elderly and disabled patients, restoring mobility and independence across outpatient care centers. This role blends direct patient care with leadership, including staff development, compliance, and operational excellence under the Rehabilitation Manager.

Qualifications

• Active California Physical Therapy license.
• Minimum of 1 year of experience with elderly or disabled populations.
• Current Basic Life Support (BLS) certification is required.

Responsibilities

• Provide high-quality physical therapy services to restore function and mobility.
• Evaluate patients and develop personalized care plans to restore functional independence.
• Deliver therapy coverage for multiple centers, in-person or remote, to support staffing and compliance.
• Participate in assessments, develop care plans, and attend team coordination meetings.
• Facilitate staff onboarding, training, and re-training to ensure service excellence.
• Contribute to data collection, analysis, and reporting for CQI initiatives.
• Lead presentations and assist in program planning and quality improvements.
• Join interdisciplinary committees to enhance patient outcomes and rehab services.
• Collaborate with quality teams and department leads to align services with organizational goals.
