Job summary

AED CONSTRUCTORES in Sunnyvale, CA is seeking a skilled Occupational Therapist to join a Skilled Nursing Facility team. You will help geriatric patients regain independence in ADLs and functional tasks through personalized treatment plans and collaborative care.

This role offers patient-centered rehabilitation and meaningful impact. Responsibilities include evaluating patients, developing plans, training in ADLs, recommending equipment, and maintaining documentation.

Qualifications

• Valid California OT license is required.
• BLS/CPR certification is required.
• Minimum 1 year of recent OT experience preferred in SNF/geriatric settings.
• Strong problem-solving and patient-care skills are essential.
• Ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Evaluate and treat patients with physical, cognitive, and functional limitations in SNF setting.
• Develop and implement individualized OT treatment plans.
• Train patients in activities of daily living (ADLs) such as dressing, bathing, and feeding.
• Recommend adaptive equipment and environmental modifications to improve independence.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and compliant documentation of patient care and outcomes.
