Job summary

AED CONSTRUCTORES is seeking an Occupational Therapist for a Skilled Nursing Facility in Concord, United States. You will assess and treat geriatric patients, creating individualized plans to restore independence in daily living activities after illness, injury, or surgery.

The role focuses on patient-centered rehabilitation, collaboration with a multidisciplinary team, and thorough documentation of progress. California licensure and BLS/CPR certification are required.

Qualifications

• Active state license as an Occupational Therapist in California.
• Current BLS/CPR certification.
• Minimum of 1 year of recent OT experience preferred, ideally in a SNF or geriatric setting.

Responsibilities

• Evaluate and treat patients with physical, cognitive, and functional limitations in a Skilled Nursing Facility setting.
• Develop and implement individualized occupational therapy treatment plans.
• Train patients in activities of daily living (ADLs) such as dressing, bathing, and feeding.
• Recommend adaptive equipment and environmental modifications to improve independence.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and compliant documentation of patient care and outcomes.
