AED CONSTRUCTORES in San Jose, CA is seeking a skilled Occupational Therapist to join a Skilled Nursing Facility. You will evaluate and treat geriatric patients, developing personalized plans to improve daily living activities.

The role requires an active California OT license and BLS/CPR. At least 1 year of recent OT experience in SNF or geriatric settings is preferred. You will collaborate with a multidisciplinary team to maximize independence and quality of life.

Competenze

• Active California Occupational Therapist license is required.
• Current BLS/CPR certification.
• Minimum of 1 year OT experience, preferably in SNF or geriatric settings.
• Ability to work collaboratively within a multidisciplinary team.

Mansioni

• Evaluate and treat patients with physical, cognitive, and functional limitations in a SNF setting.
• Develop and implement individualized occupational therapy treatment plans.
• Train patients in activities of daily living (ADLs) such as dressing, bathing, and feeding.
• Recommend adaptive equipment and environmental modifications to improve independence.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and compliant documentation of patient care and outcomes.
