Job summary

AED CONSTRUCTORES in Merced, CA is seeking an Occupational Therapist to join a Skilled Nursing Facility. You will evaluate and treat patients with physical, cognitive, and functional limitations and develop individualized OT plans to promote independence.

Responsibilities include training patients in activities of daily living, recommending adaptive equipment, coordinating with a multidisciplinary team, and maintaining compliant documentation to support patient progress and quality care.

Qualifications

• Must hold an active California OT license.
• Current BLS/CPR certification.
• Minimum of 1 year of OT experience preferred in a SNF/geriatric setting.
• Strong problem-solving and patient care skills.
• Ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Evaluate and treat patients with physical, cognitive, and functional limitations in a Skilled Nursing Facility setting.
• Develop and implement individualized occupational therapy treatment plans.
• Train patients in activities of daily living (ADLs) such as dressing, bathing, and feeding.
• Recommend adaptive equipment and environmental modifications to improve independence.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and compliant documentation of patient care and outcomes.
