Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care to seniors in skilled nursing and post-acute facilities. This onsite role evaluates residents, develops individualized treatment plans, and delivers evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers, contributing to ongoing mentorship, growth, and a supportive work culture.

Qualifications

• Current OT license to practice in Illinois.
• Experience delivering occupational therapy in senior care settings is preferred.
• Ability to work in an interdisciplinary team.

Responsibilities
