Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role focuses on evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT with state license.
• Experience in skilled nursing or post-acute settings is preferred.
• Ability to develop individualized treatment plans and deliver evidence-based interventions.

Responsibilities
