Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role entails evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Current Illinois OT license is required.
• Experience in skilled nursing or post-acute care is preferred.
• Ability to assess residents, develop individualized treatment plans and implement evidence-based interventions.
