Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering interventions to improve ADLs, mobility, cognition, and safety.

You will document in the EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist in Wisconsin.
• Experience with seniors in skilled nursing or post-acute settings preferred.
• Ability to document in EMR and collaborate with an interdisciplinary team.

Responsibilities

• Evaluate residents to determine OT needs.
• Develop individualized treatment plans.
