Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to deliver patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Responsibilities

• Evaluate residents to determine occupational therapy needs.

• Deliver evidence-based interventions tailored to seniors in skilled nursing and post-acute facilities.
• Document assessments and progress in EMR accurately and timely.
• Collaborate with an interdisciplinary care team to optimize outcomes.
• Educate families and caregivers on therapy goals, progress, and home exercises.
