Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with a geriatrics focus to work in long-term care settings, improving outcomes for residents by coordinating care with a team of providers and case managers. You will practice with independence, consulting a supervising physician as appropriate and implementing TruHealth's national care model.

Qualifications

• APRN or PA license is required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Experience with electronic health records.

Responsibilities

• Assess and manage medical and mental health needs with a proactive care plan.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications, order labs and consultations.
• Monitor treatment efficacy and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
