Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to improve long-term care outcomes. You will work with a team of providers and case managers to deliver coordinated, high-quality care for nursing home residents enrolled in Medicare Advantage plans.

You will exercise independent judgment with supervision as appropriate, treating acute and chronic conditions while adhering to federal and state regulations. Excellent benefits and growth opportunities are offered.

Qualifications

• Master's degree or higher in nursing or related field.
• APRN/NP or PA license required (FNP/AGNP/AHACNP/PA).

Responsibilities

• Assess patients' medical and mental health needs and develop a care plan.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.

Education
