Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to join our long-term care team. You will work with a multidisciplinary group to deliver personalized, coordinated health care that improves residents’ quality of life and reduces hospital visits.

As an APRN or PA, you will exercise independent judgement, collaborating with supervising physicians and facility partners to implement our national care model while complying with all regulations.

Qualifications

• FNP/AGNP/AHACNP or PA license required.
• Degree from an accredited APRN or PA program.

Responsibilities

• Assess and plan care for long-term care residents.
• Prescribe medications and order labs/diagnostics.
• Collaborate with RN case managers and physicians.
• Ensure compliance with federal/state clinical regulations.
• Provide training/advice to facility staff if needed.
