Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with primary care/internal medicine experience, preferably geriatrics-focused. You’ll support long-term care residents by coordinating care with a team of providers and case managers, delivering value-based care in nursing homes and assisted living settings.

You will independently manage acute and chronic conditions, consult a supervising physician as needed, and collaborate with facility partners to implement TruHealth’s national

Qualifications

• Degree and licenses: APRN/PA with required state licensure.
• Experience in geriatric populations or long-term care settings preferred.
• Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and create a care plan.
• Ensuring compliance with local, state, and federal agencies related to clinical services you provide.
• Prescribing medications and ordering lab work, diagnostic procedures and consultations.
• Monitoring patients’ compliance and response to treatment and adjusting plans.
• Working with RN case managers as part of an integrated care team.
