Job summary

TruHealth is seeking an advanced practice provider, either an NP or PA, with experience in primary care or internal medicine, preferably geriatrics. You will work primarily in nursing homes or assisted living communities within Medicare Advantage plans, providing personalized, coordinated care to residents.

You will collaborate with a team of providers and case managers, exercise independent judgment, and consult with a supervising physician as needed, with a strong focus on quality of life and

Qualifications

• Masters or higher in APRN or PA program.
• APRN/PA license required.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical needs and create care plans addressing barriers to care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and diagnostics.
• Monitor treatment effectiveness and adjust plans as needed.
• Collaborate with RN case managers in an integrated care team.
