Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics/primary care experience for long-term care settings. You will work with a team to deliver personalized, coordinated care to seniors enrolled in Medicare Advantage plans, improving quality of life and reducing hospital visits.

APRN/NP or PA license is required, with a Master’s degree and at least 3 years in geriatric care. The role emphasizes independent judgement, collaboration with facility partners, and compliance

Qualifications

• APRN/PA license required.
• Masters degree or higher in a relevant field.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal agencies related to clinical services.
• Prescribe medications and order lab work, diagnostics and consultations.
• Monitor patient compliance and response to treatment and modify plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
