Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with geriatrics-focused experience, to work in long-term care settings. You will deliver personalized, coordinated health care to plan members, improving quality of life and reducing unnecessary hospital visits.

APRN/NP or PA license is required, with a degree from an accredited APRN or PA program and at least 3 years in geriatric clinical nursing or rehab.

Qualifications

• APRN or PA license required.
• Degree from accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Proficiency with Microsoft Word/Outlook/Excel.

Responsibilities

• Assess medical and mental health needs and plan care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order lab work, diagnostics, and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
