Job summary

TruHealth is hiring an advanced practice provider (NP or PA) with primary care or internal medicine experience, preferably in geriatrics, to enhance outcomes for long-term care residents. The role involves collaborating with a care team to deliver personalized, coordinated healthcare within nursing homes or assisted living communities enrolled in Medicare Advantage plans.

You will exercise independent judgment for acute and chronic conditions, consulting with a supervising physician as needed,

Qualifications

• Masters degree or higher required.
• Active APRN/NP or PA licensure.
• 3 years’ clinical nursing or rehab experience in geriatric populations.
• Experience with Electronic Health Records.
• Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

• Assess and plan comprehensive care for geriatric patients.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics, and referrals.
• Monitor patient response and adjust treatment plans as needed.
• Collaborate with RN case managers as part of the care team.
