Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with geriatrics focus to enhance care for long-term care residents. You will work with a team of providers and case managers to deliver personalized, coordinated health care that improves quality of life.

The role emphasizes independent judgment in treating acute and chronic conditions, with collaboration with facility partners and health plan leadership to implement our national model of care compliant with regulations.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience; proficient with Word/Outlook/Excel.

Responsibilities

• Assess patients’ medical and mental health needs and create a care plan.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, procedures, and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of the care team.
