Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to improve outcomes for long-term care residents. You will work with a team of providers and case managers to deliver coordinated, high-quality care in nursing homes and assisted living settings enrolled in Medicare Advantage plans.

APRN or PA license is required, with 3 years in geriatric clinical nursing or rehab and EHR proficiency.

Qualifications

• APRN/PA license required
• 3 years geriatric clinical nursing or rehab experience
• Electronic Health Records experience
• Microsoft Office suite proficiency

Responsibilities

• Assess patients and create proactive care plans
• Ensure compliance with regulatory requirements
• Prescribe medications and order labs/tests
• Monitor and adjust treatment plans
• Collaborate with RN case managers and care teams
