Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to join a team delivering proactive, coordinated care in long-term care settings. You will partner with physicians, nurses, and care managers to optimize outcomes for residents enrolled in Medicare Advantage plans.

You will exercise independent judgement while consulting a supervising physician as appropriate, supporting a national model of care that complies with federal and state regulations.

Qualifications

• APRN or PA license required.
• Masters degree or higher in nursing/advanced practice.
• 3 years of clinical nursing or rehab experience in geriatric populations.
• Experience with Electronic Health Records (EHR).
• Proficient with MS Office applications (Word/Outlook/Excel).

Responsibilities

• Assess patients' medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor patient response and update care plans as needed.
• Collaborate with RN case managers as part of the care team.
