Job summary

TruHealth is seeking an advanced practice provider, either an NP or PA, with geriatric-focused primary care. You’ll work in long-term care facilities to improve residents’ outcomes through coordinated, person-centered care with a team of providers and case managers.

You will exercise independent judgement in treating acute and chronic conditions, collaborating with supervising physicians while complying with regulations. A strong benefit/pay package and career growth await.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and create a care plan.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics and consults.
• Monitor treatment response and adjust care plans accordingly.
• Collaborate with RN case managers as part of an integrated team.
